How to use this tool

Add one risk at a time: describe the risk event, rate its likelihood and its impact from 1 to 5, then name an owner and pick a treatment strategy — Accept, Mitigate, Transfer or Avoid.

The risk score is calculated for you as likelihood x impact, and it bands the entry: 1-6 low, 7-14 medium, 15-25 high. Each risk you add is also plotted on the 5x5 heat map, so the entries that need attention first are the ones that climb toward the top-right corner.

There is no single right register. Build it, read the map, and check that the risks you rated highest are the ones you treated most aggressively.
